Insurance & Payment Policy

We accept most insurance plans. We will assist in filing insurance, but your insurance is a contract between you and the insurance company. It is essential that you check on your insurance benefits.  If you are unsure about your coverage, please contact the insurance company prior to your visit with us. Certain procedures may not be covered by insurance. For these procedures, we request that you sign a waiver in the event your insurance company refuses to pay. You are responsible for the charges regardless of reimbursement from your insurance company.  Payment of deductible, co-insurance, and co-payment is expected prior to surgery. You should obtain referrals prior to your office visit if they are required by your insurance company. Visa, Mastercard, and personal checks are accepted. Please note that when you have surgery, separate bills will be sent by the Operating Room facility, Pathology, Radiology, and Anesthesiology Services.  Questions regarding their fees should be addressed to the provider of each of these services.
