Disability Forms

We are diligent to process forms accurately and completely so that payment to the patient is not delayed.  A minimum of 3 business days is required to process forms. 

The signature of the physician is required. See additional details below.

Please hand deliver your forms to our office, or have them faxed (336-387-8200). 

DO NOT GIVE ANY FORMS TO YOUR DOCTOR.

There is a $20 fee for processing all forms, including requests for office notes for continuation of benefits.  To maintain your confidentiality we do not verify insurance information over the phone.
